If you do this, several key groups of people will get to know of your practice: the managers and supervisors in the organization; the clients whom you see; and some of the referring agencies if you are required to liaise with them during or at the end of the work with a client. The managers and supervisors are often in private practice themselves and there may be times when they have more client referrals than they can handle.

There will be an organizational protocol dealing with referring agencies which may or may not include you as the counsellor. Never go outside the protocol, even if you disagree with it. If the protocol does allow direct contact between you and the referrer, then make sure your supervisor understands what is happening and agrees every step of the way. The sort of contact I have in mind is, for example, a letter to a GP saying that the work has now ®nished. Bearing in mind the need for con®dentiality, such letters should be discussed and agreed with the client.

However, the increasing use of therapists in primary care has had a domino effect on to private practice. As mentioned, many therapists work in both health care and private settings. But for therapists who choose to work privately alone, they then have to deal with the expectations of their client. It may be that a client's ®rst experience of counselling was the six sessions he or she experienced at the surgery, and is now looking to deal with more issues in greater depth. This sets up expectations of ways of working that may be completely different to the private practitioner.
